CASC license number?

I'm using the PDF that I downloaded from the SoloSprint website to register. There is a section where it asks for my CASC license number. Apparently I was suppose to receive a plastic card in the mail but I never did. I have already tried contacing Ana Marie and emailing office@casc.on.ca but to no avail.

Can anybody help?
