Machine Shop Suggestion

I'm up the creek and looking for suggestions for a machine shop to do a small fabrication job. The part I need made is a threaded sleeve, M14 x 1.5 x 35 mm LG x 18 mm OD.

Mississauga area is preferred, but I could have the finished part couriered to me.
